If you become a permanent resident then you are entitled to free healthcare if your country has an agreement with Spain , and you paid your health contributions in your home country, most EU countries fall into this category.

In this case you need to bring two copies of form E121 to Spain . These are handed in to the nearest Centro de Salud for registration. They will stamp one of the copies and return one to you. You will then receive in the post a card entitling you to use the Spanish health service. The card will contain the name of the physician that you will need to contact for treatment, and the address of your nearest health centre.

 

If you are only going to live in Spain for less than half the year then you must bring with you your EHIC (European Health Insurance Card), that replaced the old E111 form, as unless you produce this form you will be charged for treatment.

 

If you become a resident of Spain and travel to another EU country, you still need to obtain an EHIC card from your Health Centre even if you are a resident.
